PHC offers a complete and integrated continuum of behavioral health services. This allows individuals to be placed in the most appropriate level of treatment for their needs, with maneuverability to other services depending on specific situations. PHC is dedicated to ensuring that services are available, accessible and affordable.

Each facility contains a Wellness Program that includes access to exercise equipment, nutritional counseling, smoking cessation and community activities. The goal of the Wellness Program is to develop a holistic approach to healing and individual improvement. We offer our members safe and productive alternatives that will help them maintain a healthy lifestyle and increase confidence in their own ability to cope with whatever obstacles they are facing.  

PHC offers Permanent Supportive Housing Services. These services are grounded in the SAMHSA Evidence-Based Best Practices for Housing and help members obtain appropriate living accommodations. Obtaining and maintaining a secure living environment are essential to improve your state of mind and your ability to grow as a productive individual. PHC helps get a roof over our members’ heads so they can continue on a positive path toward their best selves.

PHC offers Permanent Supportive Employment Services. These services are grounded in the SAMHSA Evidence-Based Best Practices for employment and help members find and keep jobs. Job security can be difficult, but with our help, PHC members know they are supported in their search for work. PHC helps members find roles they are qualified for and will enjoy doing knowing that they are contributing to the larger community they are a part of. PHC works to integrate our members back into their communities successfully and in a way that will ultimately better that community as a whole.
